Diabetes Education

Lead instructor, Susan Riley DNP, FNP, CDE was awarded the 2011 national Innovation in Diabetes Award by the American Association of Diabetes Educators for her SMART board based Diabetes Self-Management Program.

Family Health Care Center hosts these group diabetes education classes each month and individual educational sessions as needed by our certified diabetes educator and registered dietitian (Amy Jo Riggs).

The group sessions are certified by DEAP and follow national standards and consist of a series of three classes. The group DSME/T curriculum follows the American Association of Diabetes Educators AADE7™ Self-care behaviors and was guided by the Stages of Change. The program is organized into a series of 3 two hour sessions that are designed to meet once a month for 3 months. Session one focuses on understanding diabetes and healthy eating, session two discusses exercise and medications and session three reflects on coping and prevention of complications. Monitoring and problem solving are interwoven into all three sessions. Patients can enroll at any time as the classes are offered on a revolving basis and do not have to be taken in order.
