Family Health Care Center is proud to announce that we have received Level 3 Recognition from the National Committee of Quality Assurance for their Physician Practice Connections® - Patient-Centered Medical Home™ recognition program. We were part of a pilot group in Georgia, led by the Georgia Academy of Family Physicians, to go through the process and achieve the status as a Patient-Centered Medical Home.
Practices who meet the Physician Practice Connection standards demonstrate they have established connections to information, patients, and other providers that allow them to:
- Know and use patient histories
- Follow up with patients and other providers
- Manage patient populations and use evidence–based care
- Employ electronic tools to prevent medical errors
You may ask, “What is a Medical Home?” A Medical Home is a team-based approach to providing health care. A partnership develops between the patient, his or her PCP, and a healthcare team. Together we work to coordinate the services you need to provide the best healthcare possible.
There are many advantages to the Medical Home approach, such as:
- Reduced emergency room visits as a result of us having same day appointments available as well as in house labs and radiology
- Comprehensive care for chronic conditions such as diabetes, hypertension, and hyperlipidemia
- Proactively contacting you to schedule follow-up appointments at a time that is convenient for you
- Help coordinate your follow-up and specialty appointments
- Use of our online patient portal to submit appointment requests, refill requests, etc.

Dr. Thad Riley
Thad H. Riley, MD is a board-certified family practice physician. Dr. Riley completed his B.S. Degree in Biology and Chemistry at the University of South Carolina and his MD Degree at the Medical University of South Carolina. Dr. Riley presently serves on the Georgia Board of Family Practice and has received national NCQA recognition for excellence in diabetes care. He also serves as a Clinical Professor of Family Medicine at Mercer and Georgia Health Sciences University medical schools. Dr. Riley enjoys all aspects of family medicine but has taken a special interest in the care of persons with diabetes as he became one of the first Georgia Physicians to meet the criteria for the designation of Master Diabetes Clinician.
Dr. Angela Davis
Angela Davis, MD is a board certified family practice physician. She completed her undergraduate degree in Chemistry at Augusta State University and her MD degree at the Medical College of Georgia. She completed her family medicine residency at Spartanburg Regional in South Carolina. Dr. Davis serves as a Clinical Professor of Family Medicine for Georgia Health Sciences University medical school and a clinical preceptor for Mercer University medical school. She enjoys all aspects of family medicine, but she is particularly fond of pediatrics and women's health. She has also earned the designation of Master Diabetes Clinician and achieved recognition by NCQA for excellence in diabetes care.
Susan Riley, DNP, FNP, CDE
Susan B. Riley, DNP, FNP is a board certified family nurse practitioner and Certified Diabetes Educator (CDE). Mrs. Riley completed her B.S. & Master's Degree at the University of South Carolina. She finished her Doctor of Nursing Practice with a focus on diabetes care at Georgia Southern University. She achieved recognition by NCQA for excellence in diabetes care. As a nurse practitioner, Mrs. Riley enjoys partnering in the health of all aged patients newborns to seniors. She has a special interest and background in Rheumatology and disorders of the muscular skeletal system, diabetes, pediatrics and women's health. She currently conducts the group education and visits for diabetes self-management using an interactive approach at FHCC and has received recognition for her work in diabetes education including the use of technology in classes dedicated to teaching those with diabetes about the disease and how to handle it. I have rarely called and not been able to get in to see somebody. When I have an appointment I am happy with my wait times, I hope that continues to be a priority for your office. I can predict my leave time from work and not have to use a few hours of valuable vacation time. When I need to call and leave a message, my calls are always returned. FHCC should be the role model for other offices. I don't like going to the doctor, but I don't dread going there. From the office staff to Dr. Davis I am extremely satisfied with my choice to use your office.
